Must reside in the state of Arizona, preferably within Maricopa County. Up to 10% of travel expected to the Phoenix office or for on-site visits. Monday-Friday 8am-5pm. No weekends or holidays. The MCH coordinators are responsible for all maternity and family planning education, provider education, tracking, member materials and interventions that effect our HEDIS scores. Utilizes system applications for data entry, tracking, information gathering and/or troubleshooting. - Researches member files using a variety of systems and references to obtain appropriate background information.- Monitors healthy pregnancy measures (addressing nutrition, sexually transmitted infections, HIV testing, alcohol, opioids, and substance use and other risky behaviors, measures to reduce risks for low/very low infant birth weight, recognizing active labor and others).- Monitor dangers of lead exposure to birthing parent and baby during pregnancy and how to prevent exposure, Postpartum depression, Postpartum services available and the importance of timely prenatal and postpartum care, Ways to limit labor and birth interventions as recommended by ACOG, Support resources and programs, including but not limited to Arizona Supplemental Nutrition Program for Women, Infants, and Children (WIC), the Strong Families AZ home visitation programs, the ADHS Breastfeeding Hotline, Early Head Start/Head Start, and the Birth to 5 Helpline,- Meets education goals and metrics.- Escalates issues accordingly when further action requires secondary sources or more senior staff.- Takes inbound calls when applicable/authorized.- Make outbound calls when applicable/authorized.- Coordinates with other departments according to indicated member needs.- Collects and provides data to management for root cause and issue resolution inquiries.- Completes all required documentation.- May participate in special project work for the Medical Management Department.

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

- Registered Nurse (RN) with unrestricted Arizona state license. - 3+ years of clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.-1+ year of experience with Labor and Delivery in a hospital setting or OBGYN office - Must live in the state of Arizona - 3+ years of experience using Microsoft Word and Excel required- Qualified candidates must be able to travel up to 10% of the time Reliable transportation required.
