Job Descriptions:
Salary: $48.50 - $55.90 per hour.
Higher compensation may be available for a candidate with advanced skills directly related to the applied for role.

Our Diabetes Nurse Case Manager is someone who has a passion for educating patients on diabetes, enjoys 1-1 and group settings and is looking to help patients improve their lives through short- and long-term lifestyle adjustments, while maintaining cultural sensitivity.
Job Duties:
Engage in 1-1 appointments to optimize patient treatment plans.
Facilitate group classes.
Actively participate in community outreach, including attending occasional events.
