Queens Community House is seeking a dedicated and compassionate Family Support Coordinator (FSC) to respond to community members facing a variety of challenges, including food insecurity, job loss, housing instability, social isolation, domestic violence, and health or mental health concerns. The FSC will assess incoming requests and connect individuals to the appropriate QCH programs or external resources, ensuring that each household has the support needed to thrive.
Key Responsibilities:
Introduce individuals and families to QCH’s comprehensive range of programs, including youth employment, food pantry services, adult literacy, SNAP enrollment, senior services, childcare, and more.
Collaborate with QCH program directors to assess and respond to participant needs, helping to ensure coordinated and effective support.
Provide case management and care coordination, offering intensive support when needed to guide individuals and families through their journey to stability and success.
Build and maintain partnerships with community organizations, schools, and service providers to ensure a seamless referral process for mental health, special needs, and other support services.
Facilitate workshops and conduct outreach at school sites and community events to engage new families and share available resources.
Maintain accurate records in Salesforce, track requests and referrals, and generate reports to monitor progress and outcomes.
Represent QCH Family Support Services in meetings with funders, sharing insights and data on the program’s impact.
Continuously contribute to agency-wide planning and interventions through active participation in cross-department meetings.
Qualifications:
Fluency in Spanish is required.
Bachelor’s degree in Social Work, Psychology, or a related field; or a high school diploma with 5+ years of professional experience in community-based programs.
Demonstrated experience in case management, group facilitation, and mediation.
Familiarity with best practices in family engagement and a commitment to QCH’s multi-service, settlement house approach.
Strong interpersonal skills, with the ability to handle sensitive and confidential matters, build trust, and maintain strong relationships with community members.
Highly organized and capable of managing multiple projects in a fast-paced environment.
Ability to work flexible hours, including evenings, early mornings, and weekends.
Proficiency with Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) and experience with data-tracking systems such as Salesforce.
