Care Manager, RN - Capacity Coordination Center
4 x 10 hour shifts Days, with every other Weekend
As part of a multi-disciplinary team, applies nursing knowledge and processes to facilitate patient transfers for inpatient hospitalization, between levels of care during inpatient hospitalization, and post-acute care discharge coordination. Processes require review of the medical record, extensive collaboration with hospital providers, and transportation teams using electronic and telephone communication. Capacity Coordination Center Nurses are responsible for providing a high level of customer service in a high stress, fast paced environment while maintaining flexibility and the ability to adapt, improvise and problem solve in rapidly changing conditions. .
Responsibilities
Obtains and documents relevant clinical data to individually assess the needs of each patient to ensure appropriate level of care and proper placement of patients.
Acts as a navigator for referring providers in identifying and accessing appropriate Dartmouth Health services.
Manages and coordinates communications between DH providers and other facilities via telephone and electronic communication methods.
Identifies patients and facilitates redirect and placement to DH system members. Monitors referrals to system members and collaborates with local decision makers to identify barriers and resources required to support system transfers.
In collaboration with the UM/AMD determines appropriateness of requests for inpatient transfers from outside of the DH system.
In collaboration with DHMC and DH system care management acts as a conduit to communication to address barriers to post-acute care transfers
Arranges and coordinates local, regional and international transfers to DH, for patients who require tertiary care or who have established care at DH. .
Works with medical staff to appropriately re-prioritize transfer queue. Directs, coordinates, and updates patient transfer queue.
In collaboration with the care team formulates and implements a care management plan that addresses patient/family needs, issues, resources and care goals; determining the choices available to individual patients
Communicates and collaborates with DH medical staff to prioritize activities associated with accurate transfer timing, placement needs, and bed assignment. .
Coordinates and actively participates in daily capacity huddles with On-Call Medical Director, administration, and designated system member nurse leaders to get the right patient, in the right bed, for the right care level.
Identifies and delegates appropriate tasks to assistive personnel/coordinators.
Develops and promotes long-term relationships with referring facilities and providers.
Determines and assigns accurate patient admission type following established guidelines and criteria.
Actively engages in the daily implementation of strategic goals. Engages with leadership to develop, maintain, and improve departmental policies, procedures, and workflows. Interprets and supports policies, procedures, and safety standards to facilitate daily operations.
Perform other duties as required or assigned.
Qualifications
Graduate from an accredited Nursing Program required.
Bachelor of Science Degree in Nursing (BSN) is strongly preferred.
3 years of Care Management experience strongly preferred.
Previous computer experience required (EPIC, CarePort, Word, Windows, and Excel).
Proven organizational and multi-tasking abilities along with strong customer service skills that include excellent phone and email/electronic communication etiquette.
Required Licensure/Certifications
Licensed Registered Nurse in New Hampshire required.
Basic Life Support (BLS) Certificate required.
