    Position Summary

The Lead Engagement LifeSet Specialist is responsible for outreach, engagement events, working with referral sources, and providing support to address the needs of young people 17-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood.

This temporary pilot position and will be evaluated after implementation at six months.

Essential Responsibilities

  • Conducts outreach and attends engagement events.

  • Creates and facilitates peer groups at a minimum of one time per week at other Independent Living (IL) locations (e.g., Youth Zone, Resumption).

  • Works collaboratively with all Children, Youth, and Family offices as well as other IL provides and potential referral sources regarding potential referrals or youth in need.

  • Utilizes drop-in space at other Transition Age Youth (TAY) locations (e.g., 412 Youth Zone) monthly at minimum.

  • Provides shadowing and mentoring to LifeSet Specialists as well as active outreach for referrals that are difficult to reach or engage.

  • Maintains an active caseload of five (5) youth with flexibility depending upon program staffing.

  • Serves as a back-up during staff vacations or sick time, as needed.

  • Provides mentoring and training to team members regarding referral engagement and enrollment.

  • Assist with reaching out to referral sources regarding wait times and gathering additional information.

  • Assists with the maintenance of an active referral and waiting list.

  • Assists with tracking for census management/caseload.

  • Ensures that providers and referral sources receive timely responses to their inquiries.

  • Assessment

  • To assure proper assessment the YV LifeSet Specialist will:

  • Gather and understand current and past psychosocial histories.

  • Use a strength focused, solution based approach to provide comprehensive information for a service plan on a monthly basis.

  • Assess risk factors related to safety and well-being of the young adult.

  • Provide clinical direction that meet the needs of the young adult.

  • Service Plan Development

  • To assure the development of effective treatment plan/services, the YV LifeSet Specialist will:

  • Plan and lead treatment team meetings for the purpose of creating service plan.

  • Work with team members to develop ownership for the service plan.

  • Write and utilize specific measurable goals, objectives, and time frames for each young adult.

  • Develop crisis plan for each young adult.

  • Service Plan implementation

  • To assure the implementation and evaluation of effective treatment plans that fosters goal attainment, the YV LifeSet Specialist will:

  • Deliver services outlined in the service plan

  • Monitor and record young adult’s progress.

  • Provide advocacy for the young adult to outside agencies.

  • Evaluate and reform goals and objectives as necessary.

  • Provide support to young adults in times of crisis.

  • Professional Relationships and Therapeutic Alliance

  • To assure the development and maintenance of good working relationships, the YV LifeSet Specialist will collaborate with:

  • Young adult and their natural support system.

  • Internal and external stakeholders.

  • Program team members.

  • Supervisor and organizational support staff.

  • Quality Assurance & Compliance

  • To assure adherence to quality standards and compliance with external licensing and regulatory agencies, the YV LifeSet Specialist will:

  • Comply with Pressley’s standards and state licensing requirement.

  • Exemplify the values of the program service model.

  • Complete all paperwork in a timely and professional manner.

  • Produce billable units that will meet the program’s requirement.

  • Maintain flexibility and creativity in working a non-traditional schedule to accommodate the needs of children and families.

  • To assure continuous professional growth and development, the YV LifeSet Specialist will:

  • Actively participate in individual and/or group supervision and staff meeting

  • Attend and participate in all required and other professional training

  • Independently seek out resources and learning opportunities to foster ongoing professional growth.

    Qualifications

  • Clearances: State Police, FBI, and Child Abuse Clearances; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training per state regulation.

  • Valid State Driver’s License and insurance.

    Working Conditions

  • Physical Demands: Requires vision, speech, and hearing.

  • Environmental Factors: Requires extensive travel to client homes and throughout the community. Work is performed primarily in client homes, where conditions may include tobacco smoke, domestic animals, household pests, mold and other allergens, and other conditions common to domestic environments.

  • Working Hours: A non-traditional work schedule as defined by service needs.

Qualifications

Education

Required

  • Bachelors or better in Human Services

Preferred

  • Masters or better in Human Services

Experience

Required

  • LifeSet Specialist Certification.

  • 1 year: Supervised experience in human services.

Preferred

  • 2 years: Supervised experience in human services
